Title page verso: woodcut portrait of Paré, aged forty-five, in an oval border. Paré's motto, " Labor improbus omnia vincit", appears encircling the portrait. Didot attributes the design of this portrait to Jean Cousin and states that it was engraved by Jean Le Royer, the printer of this volume.
With printed shoulder notes.
The dedication, dated 28 February, 1561, is addressed to Jean Chapelain, physician to Henri II and Charles IX.
